Industrial lift trucks are involved in nearly 95,000 workplace
accidents each year in North America. Pedestrians are injured
in about one-third of these accidents. Many of these accidents
could have been avoided by the use of a strategically located
convex mirror. Remember, a blind corner
is blind to both the pedestrian and the lift truck operator.
A properly placed convex mirror affords
each the chance to see the other’s approach.
Plant pedestrians and lift truck drivers share equal responsibility
for creating a safe work environment. There are guidelines
for each group to follow for an effective safety program.
Pedestrian guidelines
- Check the convex mirror before entering
an area where lift trucks operate, especially when from
an office area. If it appears safe, slowly enter the area
and look both ways.
- Never assume that because you don’t hear a lift
truck, one is not present. Lift trucks may not immediately
be seen or heard, especially in noisy or limited visibility
environments. A convex mirror is crucial
in these situations.
- Walk single file along aisles, facing traffic, and use
the opposite side of the aisle in which lift trucks operate.
When walking in groups, always designate one person to
be the “look-out” and check all convex
mirrors before the group proceeds.
- Only properly trained and authorized personnel should
operate lift trucks. Never move a lift truck unless you
are qualified and authorized to do so. OSHA regulations
require specific application training for all operators.
- Look out for oil spills. Lift truck engine and hydraulic
systems use oil, and the liquid may drip onto the floor
if a truck is parked in one spot for a long time. (A PIG®

- Stand a safe distance from lift trucks when a load is
raised or lowered. The higher the lift, the greater the
distance pedestrians should remain away. A truck may tip
or the load may shift and fall. Check for an overhead,
domed convex mirror for added visibility.
- Stay uphill of trucks going up or down a ramp. A pedestrian
should never get behind the truck or under the load when
the lift truck is on an incline. As always, check convex
mirrors for trucks operating around blind corners.

Operator guidelines
- Never allow passengers to ride on the lift truck. Only
use the forklift for its intended purposes. Forklifts
are not designed or intended to carry passengers.
- Install New Pig’s convex mirrors
throughout the plant to improve driver visibility. A tight
operating condition amidst racks in a warehouse or machinery
in a factory limits an operator's line of sight. An obstructed
view makes it difficult to see the entire work area. Remember,
as the operator, you know where convex mirrors
are needed. Let management know!
- Always stop and sound the horn when approaching a blind
intersection to get the attention of pedestrians and other
equipment operators. If visibility is lost, employees
need to know that the vehicle approaches. Never assume
that they see you coming in a convex mirror…they
might not be looking!
- Use extreme caution on loading docks. These areas are
hazardous for several reasons. Loading docks are highly
concentrated and in constant motion with loading and unloading
activities. In addition, wet conditions from rain or snow
are extremely hazardous, and slick surfaces can prevent
lift trucks from braking within a safe distance. Convex
mirrors are crucial in this situation!
- A lift truck must never travel towards a person in front
of a fixed object. Even though a lift truck operator has
the ability to make sudden stops, a load could fall and
strike a pedestrian after the truck stops.
- Use caution when turning to account for the lift truck's
tail-end swing. Lift trucks are equipped with a counterweight
at the rear to offset heavy loads. The rear wheels do
the steering, giving lift trucks the ability to maneuver
in tight spaces. This design causes the back end to swing
out in the opposite direction when turning. Operators
must be sure to check convex mirrors
and clear pedestrians and other objects from the direction
of tail swing when turning their lift truck.

As a final rule, both drivers and pedestrians should use
common courtesy and good judgment. Pedestrians generally
have the right-of-way, absent of specific workplace guidelines.
It is the responsibility of the lift truck operator to ensure
the safety of the pedestrians by proceeding cautiously and
checking convex mirrors along the vehicle’s
path. Pedestrians must also be aware of their environment
and its potential risks, and should know the importance
of checking convex mirrors before walking
into blind areas or around corners.
When implementing and enforcing safety regulations, assess
the requirements of each situation, whether it is the entire
plant, warehouse, loading dock, or other material handling
situations. Different conditions require different safety
plans. When evaluating the company’s specific requirements,
consult a safety professional familiar with your type of
industrial setting. Remember, if you see someone performing
an unsafe activity, it’s okay to “squeal”
on them if health and safety is at stake.
